Abstract

The present experience report describes the creation and implementation of a Consulting Program, conceived by two specialist occupational therapists, aimed at managing and monitoring healthcare for people with intellectual disabilities in the process of aging. The initiative emerged from the need to develop intersectoral and integrated strategies capable of responding to the complex demands of this population. The program focuses on strengthening the support network, ensuring continuity of care, and fostering coordination among the different sectors involved. The results indicate progress in the qualification of care, as well as in the staging of functional losses, the maintenance of independence in remaining skills, and the promotion of social inclusion.
