Abstract

Discharge of an intellectual rehabilitation program is a subject that raises many reflections. In Specialized Rehabilitation Centers, the outcome of a treatment is a practice that must consider the ethical-philosophical principles, health condition, social policy, total inclusion and the subject's quality of life. It is necessary procedures that are based on the understanding of potential and social participation, reducing the barriers that can prevent the acquisition of autonomy of the person with disability. Therefore, this manuscript presents an experience report of the construction of the discharge process at the Specialized Center for Intellectual Rehabilitation of the “Associação de Pais e Amigos dos Excepcionais” from Anápolis. Likewise, a model based on the triad – International Classification of Functioning, Singular Therapeutic Project and Rehabilitation Programs – is proposed, reflecting on the subject's needs and functionality for social inclusion. As a result, the discharge in health care services for people with disabilities is possible when considering the uniqueness of the individual and the maximum acquisition of their potential, supporting and preparing caregivers for the journey of life.
